Yes, that's the hilt of a sword. It looks like the blade was made as a separate piece with a plug hole to fit it into.
Yes, this is damage. I can hardly imagine a statue would have been commissioned of Trajan following a bar brawl in the Subura. That notwithstanding, there seems to be a number of statues of him with a mutilated nose. Perhaps he did something inappropriate with it. Who knows?
Considering their age and the chaos that surrounded the collapse of the Roman Empire, I suppose it's remarkable that we have as many undamaged sculptures from that era as we do.
