Upside down
That all over tan was probably natural because Tony Sansone was known to frequent a bath house at Coney Island that had a rooftop nude men's sun deck. I think this may have been taken there.
My sources say that Tony Sansone did this handstand pose for sculptor Chester Beach (1881-1956), who was sculpting a piece called "Sun, Mist and Sea". I did a web search for this work, but couldn't find a picture of it. This photo is by Achille Volpi, sometime before 1930.
